While preparing to go on duty, Officer Warner was transferring his police revolver from his civilian holster to his uniform holster when the weapon accidentally discharged. The bullet hit Officer Warner above his left eye. Officer Warner was rushed to the hospital and died shortly thereafter on June 1, 1981.
